An Illinois-based metal stamping manufacturer is facing a major fine stemming from an amputation incident at its El Paso factory.
OSHA said July 19 that D&D Manufacturing has been cited for two willful, 12 serious and three other-than-serious violations, totaling $412,750 in proposed penalties.
The agency inspected D&D’s plant in January after an employee had two fingers amputated inside a 500-ton hydraulic press.
The inspection revealed that D&D failed to use required machine guards, didn’t conduct regular inspections and didn't ensure employees use appropriate PPE.
OSHA’s area director said, “The willful and serious violations identified in this inspection show D&D Manufacturing Inc. continues to ignore required worker safety protections. Moving machine parts have the potential to cause severe injuries when they are not properly guarded and safety procedures are absent.”
Between 2014-2015, OSHA cited the same plant for 47 total violations.
D&D has approximately 60 employees at its El Paso plant.
