38-Year-Old Dies in Machine at Twine Manufacturer

The company makes industrial twines and tree ropes.

A 38-year-old worker died last Saturday after he was caught in machinery at PolyExcel, a company in Clearfield, Utah. 

According to KSTU-FOX 13, Jeffrey Pead suffered fatal injuries in the accident. He was pulled from the machine by first responders, but emergency personnel declared him dead at the scene.

The incident is currently under investigation, but police don't suspect foul play.

The company has not stated the type of machine or how Pead became caught in it, however in previous accidents at similar manufacturers, employees were pulled into the machine, typically as a result of a lack of machine guarding.

Local authorities did reach out to OSHA to request their assistance in the investigation. 

According to PolyExcel's website, the company is a manufacturer of baler twine that also makes industrial twines and tree ropes.  

IEN left a message for a company shift supervisor and inquired via email, but they company has not responded to our requests for comment.
